The Joint Commission Performance Measures

Effective January 1, 2010, certified primary stroke centers must collect and report on eight National Inpatient Hospital Quality Measures for stroke. These measures include:

The Gold Seal of Approval
  • Venous thromboembolism (VTE) prophylaxis
  • Discharged on antithrombotic therapy
  • Anticoagulation therapy for atrial fibrillation/flutter
  • Thrombolytic therapy administered (or documented reason why it was not given)
  • Antithrombotic therapy by end of hospital day two (or documented reason why it was not given)
  • Discharged on statin medication
  • Stroke education
  • Assessed for rehabilitation

All of these measures must be documented in EVERY stroke patients chart in order to meet quality standards for Primary Stroke Center Certification through The Joint Commission.
