The title of the template can be changed by adding |Title=My Title Goes Here (For example: {{Self assessment|Title=Self Assessment Question (SAQ)| content of the template ...}} ) in the call of the template so that the generic title of Self Assessment is preserved for all of the pages that currently link to it. Another method would be to copy and paste the template code into a new template and edit it to have a different title.
