Grace Asare is my name. I'm 30years of age and about five feet tall. I have a round face and chocolate in completion. I'm a nurse educator by profession and currently teaching at the Community Health Nurses' Training School, Winneba in the central region. I'm a Ghanaian and I come from Aburi in the eastern region. My parents are Mr. and Mrs Asare. I have four sisters and a brother. I'm the fifth of the six children. My hobbies are jogging and listening to music. ASSIGNMENT
SUBJECT: GENERAL NURSING TOPIC: VITAL SIGNS SUB-TOPIC: TAKING OF TEMPERATURE THE AXILLA
REQUIREMENT
A tray containing the follwing:
1.Thermometer in it container
2.A gallipot containing a clean cotton wool swab
3.Pen
4.Temperature chart
5.Ruler
PROCEDURE 1.Explaian procedure to the patient. 2.Send the tray to the patient's bedside. 3.Wash your hands. 4.Make the patient comfortable in bed. 5.Dry the patient's armpit with a cotton wool swab. 6.Examine the thermometer and wipe it from thebulb to the sterm. 7.Insert the thermometer into the patient's armpit making sure that the bulb is covered with the skin folds. 8.Cross the patient's arm over his chest with the palm resting on his shoulder. 9.Allow the thermometer to read for 3minutes. 10.Remove the thermometer and read it at the eye level. 11.Clean the thermometer from the sterm to the bulb. 12.Chart the reading on the temperature chart. 13.Thank the patient for his cooperation. 14.Discard the tray. 15.Wash your hands. 16.Record your findings in the nurses note. 17.Report any abnormality to the ward in-charge.
