Yes, it is easy enough to add them. The template uses the #if parser function.

- How should the template behave when different parts of the hierarchy are left blank?
- Should the template add [[Category:]] tags for different components of the hierarchy?

1. If left blank then it should omit the field - this would be most flexible 2. I think adding a category that matches the entered taxon for each of Phylum, Class, and Order would be useful. So if "Trichoptera" is added as an order, we get a "Trichoptera" category. A general "taxon" category would help gather all of the groups tagged.
