Problem with inserting a quiz inside the Activity Template

Hi Colleagues,

I encountered an interesting issue with inserting a quiz in the one of the activity templates - Template:Activity2. At first in works, as shown here:

But if I click on 'edit', it becomes messed up even if I didn't do anything to it.

Currently, it's okay...and I'm I don't want to edit my userpage again otherwise I mess up the content of Activity Template.

Even when I go to History and try to revert the recent changes, I get a message that I cannot undo them!
