Here are the items you will find in the resource pack before you start each scene. The Resource Pack can be found in the upstairs back room.

Scenario instructions - READ ME FIRST

Welcome to the Midwifery Normal Birth Scenario

Here are a few things that you will need to know and a few decisions that you need to make before you begin working through this scenario.

We suggest that you familiarise yourself with the layout of the Birthing Unit if you havent already done so. You have picked up this resource pack in the Resource Room upstairs. Go back down the stairs and have an explore - in particular you will need to find:
1. The Midwives office
2. The Treatment Room
3. The Red Birthing Room
4. The Clinic Rooms

Make sure too that you look in the cupboards and identify the different objects that will 'do' things. You will find some actions will start with you clicking on a button on your Controller HUD but some things will be activated by you clicking on the object.

RESOURCE PACK
This resource pack contains everything that you need to get started in this scenario. You should take time out to read through the various notecards and familiarise yourself with the things you need to do. It is important to your enjoyment of the scenario that you take a little time to prepare yourself. There is information here that is relevant to both the pregnant woman and the midwife and some that is specific to either one. We suggest that you read it all!

HUDS (Controllers)
You will find two HUDs in the pack - these are called the SLENZ Midwife Controller and the SLENZ Mother Controller. You will need to decide which one you are going to role play and then wear that HUD.

The scenario will not begin until you are both wearing your Controller HUD. It is then the Midwife's HUD that will determine which Scene you will be starting. So the first thing that the Midwife needs to do is to select the correct Scene button on the HUD. If your HUD seems unresponsive, you may need to click the reset button.)

PREGNANT SHAPE
Your resource pack comes with a pregnant shape and it is important that the expectant mother wears the pregnant shape. Many of the animations that the expectant mother will be using require her to be this shape and if you choose not to wear it some of those animations may not function correctly. It is also important to take off your shoes as the animations are set to work for an avatar of a particular height and shoes can interfere with that.

BIRTHING ROOM
There are two birthing rooms in the Birth Unit, the green one and the red one. They look almost identical but the green one has been set up with a number of information items, the red room has been set up for this Normal Birth Scenario. All the scenes, except the first one, take place mainly in the red birthing room.

The room has been set up to allow only one couple at a time to use the scenes. If the room is already in use then you will need to wait until it is free.

SCENES
There are five scenes in this scenario.
Scene 1 Phone call
Scene 2 Early labour
Scene3 Established labour
Scene 4 Birth of the baby
Scene 5 The first hour after birth

The scenario has been designed with the thought that you will follow through these scenes in order but you can stop at any time and return later or just pick up one of them and work on that one.

COMMUNICATION
You always have the choice in Second Life of using Voice chat or Text chat to talk to other people - it is usually a purely personal preference but you may need to take into account whether you both have access to Voice.

Decisions for you to make Once you have read through all the information, you need to make these decisions and then you are ready to start. 1. Who is the midwife and who is the expectant mother
2. Which scene are you going to start with
3. For the expectant mother to wear her pregnant shape
4. Whether you are going to use Voice chat to communicate with each other - or text chat.

Instructions on how to access the Resource Pack

Right click on the Resource Pack

This will put a folder in your inventory called "Midwifery Resource Pack" which contains all the items you will need for the scenario.

Instructions on how to wear and use the HUD

A HUD (Heads Up Display) is a special menu screen that you wear on your screen. It has a number of buttons which when you click on them will either make certain things happen (e.g. a phone rings) or will allow you to make certain actions.

In this Resource Pack you will find two different HUDS. One is for the midwife and one is for the woman. Once you have decide which role you are playing you will need to activate the relevant HUD.

Activating your HUD


All the contents of the Resource Pack will have been placed into a folder in your Inventory called Midwifery Resource Pack.
1. Locate the Midwifery Resource Pack folder in your inventory and open it.
2. Select the name of the appropriate HUD.
3. Right click and choose wear.
The HUD will appear in the centre top of your screen.

Removing your HUD


You can remove the HUD at any time but it is not a good idea to do this when you are in the middle of a scene. Make any adjustments (see below) before you start and then leave the HUD in place until the end of the scene or until the end of the scenario.

1. Locate the Midwifery Resource Pack folder in your inventory and open it.
2. Select the name of the appropriate HUD that you are wearing.
3. Right click and choose detach.
(An alternative way to find it is to open your inventory, type " (worn " into the search bar. This will return a list of all the things you are wearing which will include your HUD. You can then select it from there and right click and detach instead).

Adjusting your HUD


When you choose Wear for your HUD it will appear on your screen at the default position that we have set. If you prefer to have it elsewhere on your screen you should follow these steps.

1. Remove your HUD if you are wearing it (see above)
2.. Locate the Midwifery Resource Pack folder in your inventory and open it.
3. Select the name of the appropriate HUD.
4. Right click and choose attach to HUD.
5. Choose your preferred location on the screen from the options that you are given.

If you do not like your choice - remove your HUD and try again :)

Getting familiar with your HUD


Once you have your HUD in a place on the screen that is suitable for you, you can have a look at the buttons. Some of these will always be the same, others will only appear for certain scenes. The first button that you will need to click is the one which sets up the scene you are going to do. Both the midwife and the woman will need to click on the relevant Scene number.

The scenes are
Scene 1: Phone Call
Scene 2: Early Labour
Scene 3: Established Labour
Scene 4 : Birth of the Baby
Scene 5: The first Hour after Birth.

Each scene is a separate activity and you can undertake them in any order although it is recommended that you work your way sequentially through all 5. However you may well want to stop after one of the activities and then restart again at a later day or time.

Instructions on creating, saving, writing and passing notecards

Notecards are the most common way to write up and pass on information in Second Life. If you havent already done so - take a few minutes to watch this helpful video from Torley Linden on Creating and Saving Notecards.

As a reminder of the main points

1. Open your inventory.
2. Click on the Create tab and choose New Note
3. Rename your New Note to something meaningful
4. Write what you need to in the empty notecard (you may need to re-open it if it has closed)
5. Press the Save button
6. Close the notecard.
7. Locate the notecard in your inventory (TIP: Look under the Recent Items tab) and drag and drop it on to the person or object that you want to give it to. A copy will remain in your inventory too so that you still have a copy :)

CHANGING SHAPE


If you are role-playing the pregnant woman in this scenario you will probably want to make your avatar look as though she is pregnant. It is very easy to change your shape in Second Life but it isnt always easy to get back to your original look. So it is a good idea to save your 'outfit' (which includes all your clothes as well as your shape, your hair etc) before you start changing into new shapes.

As usual there are some very good videos to help you and it is a good idea to watch them before you start. Although they talk about your library shapes everything in the video also works for other shapes and outfits that you have in your inventory.

As a reminder of the main points.

Saving your current 'outfit' 1. Click on yourself and choose Appearance
2. Click on the Make Outfit button
3. Give your 'outfit' a name (e.g. MeNormal)
4. Tick all the parts you want in your 'outfit'
5. Click OK

This 'outfit' is now saved in your inventory as a folder called e.g. MyNormal. You only need to save this outfit once. A copy will stay permanently in your inventory.

To change back to this outfit
1. Locate the folder in your inventory
2 .Right click on it
3. Choose Replace Outfit

It may take a few moments but it will replace your current shape and clothes with the new outfit that you have selected. Wearing the Pregnant Shape


1. Save your normal 'outfit' if you haven't done so already (see above)
2. Locate the pregnant shape in your inventory
3. Right click on the shape and choose wear

It may take a few moments but it will replace your current shape with your new pregnant shape.

Instructions on how to communicate

You usually have the choice in Second Life of using Voice chat or Text chat to talk to other people - it is usually a purely personal preference but you may need to take into account whether you both have access to Voice.

Using either text or voice chat you have the option of using either Local Chat or Private Instant Message. For the Normal Birth Scenario we suggest that you and your partner use Private IM whether you are using voice or text. That way no one else will be able to hear/see what is being said.

IM
The easiest way to begin an IM session is to click on the avatar you wish to talk to and choose Send IM. This will open your Communicate window and place your cursor in the message box at the bottom of the window. If you look you will see that the tab at the bottom holds the name of the avatar that you have selected. The IM will now only go between the two of you. If you wish to add more people to this private IM session you can set up a Friends conference. However all the people that you wish to add to the call must be your 'Friends' - that is they must be on your Friends list.

FRIENDS CONFERENCE
To set up a Friends Conference.
1. Click on the Communicate button at bottom right of your screen.
2. Click on tab Contacts and then Friends
3. Click to select a Friend
4. Cntl-Click to select the next Friend (continue until you have them all selected)
5. Press the IM/Call button
6. A new communicate window opens with the tab Friends Conference. (They can now all talk to each other)

Instructions for mother on how to wear clothes

Your Resource Pack has given you a set of PJs ( baggy pyjamas) to change in to at some point during the scenario (when and if you change is up to you!).

Changing clothes is easy in Second Life and there are various ways to do it. If you have already found one you like, then stick with it! If not, here is one way to do so.

Find out what you are currently wearing.
1. Open up your inventory
2. Type "(worn" - without the quote marks - into the search bar at the top of your inventory window.
3. Now you can see all the things you are wearing. (Right clicking on any item will give you the option to either Take Off or Detach from Yourself, this item from your avatar.)

Wearing the Clothes
1. 1. Save your normal 'outfit' if you haven't done so already
2. Locate the PJ Top in your inventory
3. Right click on it and choose Wear
4. Do the same for PJ Pants.

Removing your Shoes
For the animations to work properly with your pregnant shape you will need to remove your shoes.
1. Open up your inventory
2. Type "(worn" - without the quote marks - into the search bar at the top of your inventory window.
3. Identify the shoes you are wearing
4. Select one of your shoes and right click - choose Take Off or Detach from Yourself
5. Repeat for the other shoe.
Some shoes come with a shoe base. Check to see if yours do and if so remove them the same way as the shoes.

This is a list of the items in the birthing unit that you may need to use in the Normal Birth scenario. It is a reference list only.
Please note that some items will only respond when you are in the scene that requires them.

Scenario: Normal Birth

Instructions for Scene One - Phone Call

Instructions

BACKGROUND This scene focuses on the phone call that Lisa who is in early labour makes to the midwife in the birthing unit. The aim of this scene is for the midwife to assess the woman's condition over the phone, make a judgment about what is the right course of action to take and shares appropriate information.

START: The scene starts when Lisa calls the midwife.
FINISH: The scene ends when the midwife is satisfied with her assessment of the situation and the plan of action that has been developed, and Lisa is happy that all her questions have been adequately answered.

MIDWIFE
1. Wear the Midwife's HUD
2. Choose Scene 1 and wait for the HUD to reset.
3. Go to the examination room and wait for the phone call.
4. When you hear the phone ring - click on the phone to 'answer' it (i.e. stop it ringing)
5. Look for the IM chat to you from your partner - this may be voice or text depended on what you have decided.
6. Listen to what Lisa has to say, make your assessment and advise Lisa accordingly.
7. Click on the Notes button on the HUD. You will be given a notecard to complete with your documentation.
8. When you have finished writing up your notes, save and close the notecard.
9. Locate the notecard in your inventory - look in Recent Items, it is called ACT1 MW Notes - drag from your inventory on to the filing cabinet in the office.
10. Keep an eye open for the feedback notecard that will come to you from Lisa. 11. You will now have another active button on your HUD for your self-assessment. CLick on the button to receive another notecard for you to write up your self assessment. (This notecard is called ACT1 MW Self Assessment)

LISA
1. Wear the Mother's HUD
2. Choose Scene 1 and wait for the HUD to reset
3. Click on the Script button on your HUD and read the suggested script for this scene.
4. Stay in the meeting room on the first floor of the birthing unit and sit down so you are out of sight of the midwife.
5. Click the phone button on the HUD. This will start the phone ringing and the Midwife will hear it.
6. Start an IM conversation with the midwife. You can choose whether to use voice or chat.
7. Use the script until you are satisfied you have had all your questions answered.
8. CLick on the Feedback button on your HUD - this will give you a notecard to complete as feedback for the midwife.
9. When you have finished writing your notecard, save and close it.
10. Locate the notecard in your inventory (the notecard is called ACT1 MTH Assessment) and drag a copy on to the midwife's avatar (this will give her a copy). The notecard will remain in your inventory if you want to look at it later.

Instructions for Scene Two - Early Labour

BACKGROUND This scene focuses on the admission of Lisa, who is a pregnant woman in labour, to the birth unit. The aim of this scene is for the midwife to welcome Lisa to the birth unit, assess her condition and make a plan of action for advice and support over the next few hours.

START: The scene starts when the midwife meets Lisa in the foyer of the birth unit.
FINISH: The scene ends when the midwife is satisfied with her assessment of the situation and the plan of action that has been developed, and Lisa is happy that all her questions have been adequately answered.

MIDWIFE
1. Wear the Midwife's HUD
2. Choose Scene 2 and wait for the HUD to reset.
3. Go to the foyer of the birth unit and wait to meet Lisa.
4. Look for the IM chat to you from your partner - this may be voice or text depended on what you have decided.
5. Listen to what Lisa has to say, carry out your clinical assessments and midwifery actions, make your plan of action for the next few hours and advise Lisa accordingly.
6. Click on the Notes button on the HUD. You will be given a notecard to complete with your documentation.
7. When you have finished writing up your notes, save and close the notecard.
8. Locate the notecard in your inventory - look in Recent Items, it is called ACT2 MW Notes - drag from your inventory on to the filing cabinet in the office.

Instructions for Scene Four - Birth of Baby

BACKGROUND This scene focuses on the care of Lisa, who is in labour and about to move into the second stage of labour and birth her baby. The aim of this scene is for the midwife to assess Lisa's condition and support her as she has her baby.

START: The scene starts in the red birth room in the birth unit when Lisa complains that she is in a lot of pain and is starting to feel like she wants to push.
FINISH: The scene ends when the baby is born but before the birth of the placenta and membranes; the midwife is satisfied with her assessment of the situation and the plan of action for the next hour has been developed; and Lisa is happy that she knows how to care for her new-born baby.

MIDWIFE
1. Wear the Midwife's HUD
2. Choose Scene 4 and wait for the HUD to reset.
3. Go to the red birth room in the birth unit and meet Lisa.
4. Look for the IM chat to you from your partner - this may be voice or text depended on what you have decided.
5. Listen to what Lisa has to say, carry out your clinical assessments and midwifery actions, make your plan of action for the next few hours and advise Lisa accordingly.
6. Click on the Notes button on the HUD. You will be given a notecard to complete with your documentation.
7. When you have finished writing up your notes, save and close the notecard.

Instructions for Scene Five - first hour after birth

BACKGROUND This scene focuses on the care of Lisa who has just had her baby, for the first hour after the baby has been born. The aim of this scene is for the midwife to assess Lisa's condition and support her as she births her placenta and membranes, and cares for her newborn baby.

START: The scene starts after the baby has been born but before Lisa has birthed her placenta and membranes. FINISH: The scene ends when the third stage is complete and Lisa and the baby are comfortable, and Lisa is happy that she knows how to care for her new-born baby.

MIDWIFE
1. Wear the Midwife's HUD
2. Choose Scene 5 and wait for the HUD to reset.
3. Go to the red birth room in the birth unit and meet Lisa.
4. Look for the IM chat to you from your partner - this may be voice or text depended on what you have decided.
5. Listen to what Lisa has to say, carry out your clinical assessments and midwifery actions, make your plan of action for the next few hours and advise Lisa accordingly.
6. Click on the Notes button on the HUD. You will be given a notecard to complete with your documentation.

Woman's pregnancy notes

These notes will give both midwife and mother information about the mother's pregnancy. Here is the link to the notes.

These notes are modeled on the pregnancy notes used by the Midwifery and Maternity Providers' Organisation, which is affilated with the New Zealand College of Midwives.

Partogram - for use by midwife

This is the link to a partogram that you can download and complete with every scenario. This is not a compulsory activity but rather an opportunity to 'practice' using this particular tool.

Many midwives would argue that a partogram is an inappropriate document to use in a birth unit in the context of normal birth. However, when labour deviates from normal and it looks as if transfer to hospital is required, a partogram can be a useful tool for sharing information with other midwives and obstetricians at handover.

Background story for woman

Your name is Lisa Smith and you are expecting your first baby. You are 2 days overdue. You are 27 years old and work as a manager at the local ‘The Storehouse’ store. You have been working up until the last 2 weeks. You own a small 2-bedroom house but are a little anxious about how you will afford your mortgage now you have given up work. You will probably go back to work part time but want to wait and see how you feel in terms of when you go back. Your husband’s name is Jack and he is a builder. You are non-practicing Catholics and Pakeha European ethnicity. Jack and you have been high school sweet hearts and trying to have a baby for three years so you’re very excited about being pregnant at last. At the same time, you’re also quite anxious. You want to be the perfect mum.

Your pregnancy has been normal. You have had a little swelling of your feet over the last couple of weeks especially when you’ve been on your feet all day. Your hands are a little swollen – you have had to take your wedding ring off. Your blood pressure went up a little at your last ante natal visit but settled really well when you had rested. You are not a vegetarian but you don’t eat a lot of red meat. You probably eat more junk food than you should.

You are expecting one baby who has been growing well. Baby has been moving well except he/she has been quite quiet over the last 12 hours or so. You think you have had at least 10 movements over the last 12 hours but it's difficult to say because you have been focusing on your contractions. All your blood tests have been normal. You had a scan at 20 weeks pregnant which showed that everything appeared to be normal.

Your husband is away on work at the moment so you will be relying on the midwife for support when you go into labour.

When you role play the woman, you will be asked to give feedback to the midwife about her performance at the end of each scenario.

  • How easy was the midwife to talk to?
  • Was the midwife sensitive to your needs?
  • How satisfied were you with the information the midwife gave you?
  • How satisfied were you with the way the midwife listened and responded to your questions and concerns?
  • Were your decisions respected by the midwife?
  • How satisfied with you with the action plan that the midwife developed with you?
  • Any other comments?
